CASE MANAGEMENT

Where there’s a need for multiple forms of support, there’s also a need for cohesion. Imagine you have a therapist, a coach and a psychiatrist, and that each of them are pins on a map. Case management is the thread that connects them.

We believe in a ‘joined up’, systemic approach to behavioural and mental health. Our case management team source, coordinate and collaborate with all professionals involved. We’ll create and oversee a comprehensive treatment plan, tailored specifically for you and centred around clear goals that are reviewed regularly. Case management support extends beyond the individual receiving treatment, providing a point of contact for family members and other parties that might be involved.

From choosing the right team of professionals to coordinating support and monitoring your progress, case management is designed to ensure your treatment stays on track.

BEHAVIOURAL HEALTH COACHING

Our behavioural health coaches provide personalised, emotional and practical support for individuals experiencing mental health struggles, addiction, and behavioural challenges.

Coaches aren’t clinicians; they’re trusted allies who walk alongside you, offering guidance, understanding, and encouragement. With their lived experience and deep knowledge of recovery, our coaches bring hope and connection, helping you navigate the practical aspects of recovery.

Working collaboratively with other professionals, our coaches focus on the day-to-day details that build a strong foundation for meaningful, lasting progress. Whether it’s helping with transportation, identifying accessible resources, developing structure and purpose, social support, navigating transitions and change, household tasks, or offering intensive and live-in assistance, our coaching is always tailored to your unique needs.

Supervised by our clinical team, coaches provide the hands-on support you need to stay on track while fostering direction, independence and confidence.

Let us help you move forward with care and connection.

THERAPY

Our psychotherapists adopt a holistic and integrative approach, allowing them to design sessions around your specific experiences and therapeutic goals.

Offering in-person appointments in Lisbon, or remotely via video - we offer individual, child, adolescent, couples, family, and group sessions - all delivered from our ethos of empathy, hope and expertise.

We offer a range of evidence-based therapies, including:

●  Psychodynamic Therapy

●  Humanistic Therapy

●  Experiential Therapy

●  Cognitive Behavioural Therapy (CBT)

●  Dialectical Behavioural Therapy (DBT)

●  Polyvagal-Informed Listening Therapy (SSP)

Let us help you take the next step on your journey to healing and growth.

How does it work?

01.
Assessment
Depending on what services we recommend the next step is a formal assessment either with a clinical psychologist or a member of the therapy team.
01.
Define a path to greater wellness
You’ll meet the team working with you and chart a plan to help tackle the problems you are facing. The length of time you work with us will be driven by you and your needs – we will support you every step of the way.
01.
Initial consultation
Speak to a member of our team, in confidence, so we can better understand your needs, how we might be able to help and discuss next steps.
01.
Ongoing care
The path to better mental health and wellness is a life-long process. Our aim is that you will get to a point where you no longer need us. However, our team will always be available to provide ongoing support, advice or additional help should you need it.

Some common questions about PBH

Do you accept medical insurance?

We do not accept medical insurance.  In some instances, clients are able to reclaim the cost of our services from their insurance companies but we do not deal with insurance companies directly. If additional information is required by a client in making an insurance claim an administration fee is payable.

Are you CQC registered?

The Care Quality Commission (CQC) oversees clinical and medical support in the UK.  Portobello Behavioural Health doesn't provide clinical or medical services and so we are not CQC registered. Therapists are registered with the British Association of Counselling and Psychotherapy, The Heath and Care Professions Council or a similar accrediting body. Case management and coaching are unregulated, non-clinical services. As such there is no governing or regulatory body that oversees this work. Any concerns, issues or complaints should be addressed to admin@portobellobh.com.

What do I do in an emergency?

Dealing with mental health problems is turbulent and characterised by repeated crises. PBH is not an emergency service. If you find yourself in a situation where someone needs medical assistance or there is an imminent danger you should call emergency services. If the situation is contained and you require rapid response in the form of case management or coaching please get in touch.

Can you refer me to a treatment centre/psychiatrist/psychologist?

We work with a variety of providers and professionals in the addiction and mental health field. We often work alongside these professionals to provide wrap-around care. If PBH is not the right service for you, or we feel we are not the right fit for you we will happily make a recommendation to another provider that may be able to help.

Do you do therapy online?

Yes. We have a number of therapists who specialise in working with clients online. Get in touch and we can match you up with the most suitable member of the team.

Do you do online coaching?

Yes. We are able to support clients online. This works better in some instances than other, usually depending on the presentation and the person involved. Get in touch and speak to a member of the team and options can be discussed.

Do you have a coach who suits my lifestyle/background/sexuality/gender/age/ethnicity etc

We have a wide range of coaches we work with from a whole range of backgrounds. It is rare that we are not able to find a match that suits our clients. If a client is particularly nervous we can arrange a free introductory meeting to see whether it will be a good fit before starting to work with someone.

Do you provide confidential support?

Discretion and confidentiality are our bywords. We protect our clients confidentiality rigorously and have processes in place to ensure that information is safeguarded. We pride ourselves on our discretion. Our flexible approach to support means that we can often work with clients in a way which suits their needs for privacy.
