Deciding to get help is the first step in an often confusing process. Case management can help navigate these difficult choices.
Choosing whom to entrust your loved one’s care with and deciding what kind of support they need can be a critical decision in someone’s journey to better mental, emotional and physical health. There are many different levels of care and thousands of options open to an individual with different modalities, approaches and price points.
It is extremely important to match someone with the right care provider/s and then critically to ensure that they do the job that is required of them. Without professional training and experience it can be hard to know which criteria to go by in order to make the best choices.
Portobello Behavioural Health specialises in guiding families through this process, this is what we call case management. We have visited and vetted a huge number of providers worldwide and keep up to date with the quality of the providers and so are able to help make the right choices for the person in need of help.
We help clients and their families to navigate the often difficult decisions needed in order to achieve the right support, care and wrap-around arrangements which deliver better outcomes. We have unrivalled contacts with therapists, psychiatrist, behavioural health specialist and treatment centres in the UK which ensures that issues can be identified quickly, referrals are speedy and the right support is put in place.
Our case management also ensures that different support services are joined-up and families are kept informed about progress, challenges and achievements as we work with them. In additional to providing holistic case management oversight and care for our clients we also offer:
- Recovery coaching – for some people having someone be with them, all or part of the time, to assist and support them in their recovery can be vital. We pride ourselves in matching people with recovery coaches who not only understand recovery but understand the lifestyle needs of our clients.
- Continuing care planning and placement – we often support clients with primary treatment placements but this is only the first step. It is vital to make the correct decisions about what happens post treatment to ensure that your loved one continues to benefit and heal. Whether it is wrap around care, companionship, sober living, 12 step meetings or a therapist we work with all involved to ensure a smooth transition.
- Transporting – should someone need to get safely from their home to where they are accessing treatment or support we can provide a professional to travel with them and handle any issues along the way as well as assisting with the admissions process.
- Treatment planning – we ensure that the care provider is delivering the correct support to your loved one, we also act as a communication link between provider, family, employer, primary therapist and doctors.
